The Southport Inquiry Report was published yesterday. I’ve read it.
What it describes is not primarily a failure of intelligence gathering.
It is a failure of ownership, process, and information sharing.
📍 **From 2019, this is what was already known:**
🔹 He asked what to do if he wanted to kill somebody
🔹 He took a knife to school around ten times — and said he would use it to kill
🔹 He called the Manchester Arena attack a “good battle”
🔹 He was searching for school shootings, graphic violence, and beheadings
🔹 He was already coming to notice for violent interests and weapon-related behaviour
And then, aged thirteen:
⚠️ Taxi booked in advance
⚠️ Weapon carried
⚠️ Stated intent to kill
He carried out a premeditated attack on a fellow pupil.
That is not ambiguous.
That is:
➡️ **expressed intent**
➡️ **demonstrated capability**
➡️ **actual violence**
All before he turned fourteen.
📉 **Over the next five years, the picture did not improve. It deteriorated.**
– More weapons
– More fixation
– More disengagement from support
And throughout all of it:
❌ Risk was passed between agencies
❌ Information was lost or diluted
❌ Behaviour was too often explained away
Until, just months before three children were murdered:
🛑 a formal assessment recorded **“no risk to others.”**
🧭 **Sir Adrian Fulford identifies five fundamental failures.**
Among them:
▪️ No agency took ownership of the risk
▪️ Information was poorly managed and poorly shared
▪️ Behaviour was too readily excused on the basis of diagnosis
▪️ Online activity was not properly overseen
▪️ No one held the full picture and acted on where it was heading
🎯 **That is exactly the gap that Behavioural Threat Assessment and Management is designed to fill.**
BTAM does not start with ideology.
It starts with behaviour.
It asks one question:
👉 **Is this person moving towards violence?**
It looks at:
– intent
– capability
– fixation
– preparation
– leakage
Not as isolated incidents, but as a pattern with direction.
And crucially:

📌 one owner
📌 one accountable decision-maker
No more passing the file.
📌 The Inquiry calls for structural change, including a single dedicated agency or structure to hold responsibility for monitoring, coordination, information-sharing and intervention.
The methodology to support that already exists.
What has been missing is the will to apply it properly and consistently.
That has to change.
This was not unforeseeable.
The pathway was visible.
The real question was never whether enough was known.
It was whether anyone was prepared to act on what was known.
They were not.
And three little girls are dead.
